[ES] Se denomina grasa epicárdica al tejido adiposo localizado entre el pericardio visceral y epicardio. Se trata de un depósito de tejido adiposo metabólicamente activo que se encuentra en el espacio pericárdico. Existen diferentes estudios donde se ha concluido que un mayor contenido de tejido adiposo epicárdico está relacionado con diferentes patologías como podría ser la estenosis aórtica.
La estenosis aortica consiste en un engrosamiento y calcificación de los diferentes velos de la válvula aórtica, lo que impide que tenga una apertura normal, limitando la salida de la sangre del corazón y produciendo sintomatología limitante y aumento de la mortalidad. Por ello, estos pacientes clásicamente se han tratado mediante cirugía de sustitución de la valvular aórtica mediante esternotomía abierta, o bien, mediante un implante percutáneo de prótesis valvular aórtica (TAVI). En este último caso la intervención no necesita de esternotomía ni de ciertas complicaciones derivadas de la cirugía tradicional, lo que supone un beneficio para muchos pacientes.
Debemos destacar como biomarcadores de enfermedades cardiovasculares, el NTproBNP y CA125. El NTproBNP es un péptido natriurético que se produce en el corazón y se libera en la sangre cuando el corazón está sobrecargado o dañado, y su presencia se utiliza en medicina para evaluar la función cardíaca y diagnosticar diferentes enfermedades cardiovasculares como la estenosis aortica. Por otro lado, se ha visto que el CA125 podría estar relacionado con la inflamación sistémica y la respuesta inmune, que también juega un papel importante en algunas enfermedades cardiovasculares.
Aunque no está clara la relación entre la cantidad de tejido adiposo epicárdico y el diagnostico de enfermedades cardiovasculares, a lo largo de este trabajo se pretende realizar un estudio para explorar la relación entre la cantidad de grasa y el pronóstico de enfermedad y así poder mejorar la estimación del riesgo de los pacientes y, en última instancia, mejorar su pronóstico y calidad de vida.
Por ahora, la cuantificación de la grasa epicárdica está limitada a series de Tomografía Computarizada (TC) sin contraste, sin embargo, este tipo de series no se adquieren en todos los pacientes, mientras que las series con contraste sí, es por esto por lo que la posibilidad de cuantificar dicha grasa en este último tipo de adquisiciones es de gran interés, pues podría llevarse a cabo en un mayor número de pacientes. Además, estas imágenes con contraste presentan numerosas ventajas frente a las sin contraste, entre las que destaca la mayor resolución espacial que presentan o la mejora en la visualización de los vasos sanguíneos.
Para llevar a cabo la cuantificación de la grasa epicárdica es de especial importancia la umbralización que establecer en cada tipo de imagen. A partir de diferentes estudios se conoce que, en el caso de las series sin contraste, el umbral óptimo para la segmentación de la grasa epicárdica se encuentra entre -190 HU y -30 HU, mientras que en el de las series con contraste aún no está completamente claro cuál es el valor más óptimo, pero algunos de los umbrales más estudiados son (-190 HU, -30 HU) o (-190 HU, -3 HU).
El objetivo principal de este Trabajo Fin de Grado será pues la cuantificación, mediante segmentación, de la grasa epicárdica tanto en las imágenes sin contraste como en las imágenes con contraste de los mismos pacientes. Tras esto calcularemos un índice de semejanza entre ambas segmentaciones, lo que nos permitirá determinar si la herramienta EPIFat, desarrollada previamente en MATLAB y que permite la segmentación semiautomática y cuantificación de grasa epicárdica, funciona correctamente para ambos tipos de imágenes. Este trabajo nos permitirá validar también el uso de esta herramienta con imágenes de Tomografía Computarizada (TC) con contraste.
Asimismo, se llevará a cabo un estudio estadístico para la evaluación de diferentes biomarcadores de fragilidad y pronóstico, destacan

[EN] Epicardial fat is adipose tissue located between the visceral pericardium and epicardium. It is a deposit of metabolically active adipose tissue located in the pericardial space. Different studies have concluded that a higher content of epicardial adipose tissue is related to different pathologies such as aortic stenosis.
Aortic stenosis consists of a thickening and calcification of the different leaflets of the aortic valve, which prevents it from opening normally, limiting the outflow of blood from the heart and producing limiting symptoms and increased mortality. For this reason, these patients have typically been treated by aortic valve replacement surgery by open sternotomy or by percutaneous implantation of an aortic valve prosthesis. In the latter case, the intervention does not require sternotomy or certain complications derived from traditional surgery, which is a benefit for many patients.
We should highlight NTproBNP and CA125 as biomarkers of cardiovascular disease. NTproBNP is a natriuretic peptide that is produced in the heart and released into the blood when the heart is overloaded or damaged, and its presence is used in medicine to evaluate cardiac function and diagnose different cardiovascular diseases such as aortic stenosis. On the other hand, it has been seen that CA125 could be related to systemic inflammation and immune response, which also plays an important role in some cardiovascular diseases.
Although the relationship between the amount of epicardial adipose tissue and the diagnosis of cardiovascular disease is not clear, the aim of this study is to explore the relationship between the amount of fat and the prognosis of disease in order to improve the risk estimation of patients and, ultimately, to improve their prognosis and quality of life.
For now, the quantification of epicardial fat is limited to non-contrast computed tomography (CT) series; however, this type of series is not acquired in all patients, whereas contrast-enhanced series are, which is why the possibility of quantifying this fat in the latter type of acquisitions is of great interest, as it could be performed in a greater number of patients. In addition, these contrast-enhanced images have numerous advantages over non-contrast images, among which the greater spatial resolution and the improved visualization of blood vessels stand out.
In order to carry out the quantification of epicardial fat, it is especially important to establish a threshold for each type of image. From some studies it is known that, in the case of non-contrast series, the optimal threshold for epicardial fat segmentation is between -190 HU and -30 HU, while in contrast series it is still not completely clear which is the most optimal value, but some of the most studied thresholds are (-190 HU, -30 HU) or (-190 HU, -3 HU).
The main objective of this End of Degree Project will therefore be the quantification, by segmentation, of epicardial fat in both non-contrast and contrast-enhanced images of the same patients. After this we will calculate a similarity index between both segmentations, which will allow us to determine if the EPIFat tool, previously developed in MATLAB and which allows the semi-automatic segmentation and quantification of epicardial fat, works correctly for both types of images. This work will also allow us to validate the use of this tool with contrast-enhanced computed tomography images.
Likewise, a statistical study will be carried out for the evaluation of different biomarkers of fragility and prognosis, highlighting NTproBNP and CA125.
For this purpose, CT images of 96 patients who underwent percutaneous aortic prosthesis implantation will be used to validate the EPIFat tool for images with contrast, as well as the subsequent statistical study for the evaluation of different biomarkers.
