dc.description.abstract | [ES] El ictus es la segunda causa de muerte a nivel global y el principal causante de discapacidad en los seres humanos. El ictus isquémico, causado por la oclusión de una arteria que impide la llegada del flujo sanguíneo al cerebro, es el más común. Gracias a la terapia endovascular se puede recuperar el trombo ocluido en el cerebro restableciendo el flujo sanguíneo y permitiendo analizar el material biológico. El correcto diagnóstico y clasificación del ictus isquémico es crucial para garantizar un buen pronóstico al paciente. Con el avance de las nuevas tecnologías como la Inteligencia Artificial (IA) se pueden crear nuevas herramientas diagnósticas que facilitarían a los clínicos esta difícil tarea. En el contexto del ictus isquémico agudo hay diferentes subtipos de ictus isquémicos dependiendo del origen del trombo. Destacan los de etiología aterotrombótica, cardioembólica y criptogénica (causa indeterminada). En el presente trabajo de fin de máster se han utilizado técnicas de IA, concretamente Deep Learning, para crear dos redes neuronales convolucionales, una para la segmentación de trombos cerebrales y otra para su clasificación. Se utilizaron los datos de 166 pacientes cuyo ictus fue de etiología aterotrombótica (n = 31), cardioembólica (n = 87) y criptogénica (n = 48). Para la red de segmentación se emplearon las imágenes de las 3 etiologías y para la clasificación se utilizaron exclusivamente los trombos de etiología aterotrombótica y cardioembólica. La red neuronal de segmentación presentó un índice DICE medio del 0.95 y la mejor red neuronal de clasificación mostró 96.6% de exactitud clasificando correctamente los trombos en dos categorías, aterotrombóticos y cardioembólicos. Se creó una base de datos de 6 variables clínicas de cada paciente que fueron introducidas la red neuronal de clasificación. Las variables obtenidas fueron: edad, sexo, tabaquismo, hipertensión arterial, diabetes, dislipidemia. Posteriormente, se realizó un estudio estadístico de las variables citadas anteriormente entre los pacientes aterotrombóticos y cardioembólicos y se obtuvieron diferencias significativas en el sexo (p <0.05), el tabaquismo (p < 0.05) y la edad (p < 0.0001). Paralelamente se realizaron cortes histológicos de 20 trombos, 10 de origen aterotrombótico y 10 cardioembólico para estudiar su contenido de glóbulos blancos con la tinción hematoxilina y eosina. No se observaron diferencias significativas entre ellos. Este enfoque innovador nunca se ha realizado previamente y ha mostrado unos resultados prometedores para ser una potencial herramienta que ayude al diagnóstico clínico. | es_ES |
dc.description.abstract | [EN] Stroke is the second leading cause of death globally and the leading cause of disability in humans. Ischaemic stroke, caused by the occlusion of an artery that prevents blood flow to the brain, is the most common. Endovascular therapy can retrieve the occluded thrombus in the brain by restoring blood flow and allowing biological material to be analysed. The correct diagnosis and classification of ischaemic stroke is crucial to ensure a good prognosis for the patient. With the advancement of new technologies such as Artificial Intelligence (AI), new diagnostic tools can be created to make this difficult task easier for clinicians. In the context of acute ischaemic stroke, there are different subtypes of ischaemic stroke depending on the origin of the thrombus. Those of atherothrombotic, cardioembolic and cryptogenic (undetermined cause) aetiology stand out. In this master's thesis, AI techniques, specifically Deep Learning, were used to create two convolutional neural networks, one for the segmentation of cerebral thrombi and the other for their classification. Data from 166 patients whose stroke was of atherothrombotic (n = 31), cardioembolic (n = 87) and cryptogenic (n = 48) aetiology were used. Images of all three aetiologies were used for the segmentation network, and only atherothrombotic and cardioembolic thrombi were used for classification. The segmentation neural network had a mean DICE index of 0.95, and the best classification neural network showed 96.6% accuracy in correctly classifying thrombi into atherothrombotic and cardioembolic categories. A database of 6 clinical variables for each patient was created and entered into the classification neural network. The variables obtained were age, sex, smoking, hypertension, diabetes, and dyslipidaemia. Subsequently, a statistical study of the variables mentioned above was performed between atherothrombotic and cardioembolic patients, and significant differences were obtained in sex (p < 0.05), smoking (p < 0.05) and age (p < 0.0001). At the same time, histological sections of 20 thrombi, 10 of atherothrombotic and 10 of cardioembolic origin, were taken to study their white blood cell content with haematoxylin and eosin staining. No significant differences were observed between them. This innovative approach has never been performed before and has shown promising results as a potential tool for clinical diagnosis. | es_ES |
